In consideration of the services to be rendered by the Pat Walker's Figure Perfection Machines at Sisters, exercisers must certify that they are not at the present time under any disability that would prevent them receiving such services. Exercisers are not being attended by any physician for any ailment which would be aggravated by such treatment, nor have exercisers been informed by any physician not to take such treatment.

In the event that exercisers should claim any physical disability during their course of treatments, exercisers agree to submit to a medical examination by a licensed physician selected by the Pat Walker's Figure Perfection Salon at the exercisers own cost.

In order that we may give each patron the best possible service in Pat Walker's Figure Perfection Salons, we require that exercisers agree to conform to the following rules:

1. Either cover hair or bring a towel to place under head while exercising.

2. Understand the conditions of the agreement.

3. Do not walk barefoot in the salon at any time.

4. Wear socks at all times when taking my treatments. I understand that this is for my own protection as well as the protection of all other patrons.

5. Give a two-hour cancellation notice for any of my scheduled appointments.
